By increasing access to diabetic-appropriate staple foods and fresh produce, self-management educational tools, and connections to local healthcare providers, the DWP has resulted in reduced blood sugar levels, increased vegetable intake, and improved the overall health of participants. The program also reduces the stress associated with food insecurity and managing diabetes, giving our most vulnerable neighbors a chance to thrive.
An alarming 59% of Sonoma County adults are either pre-diabetic or diabetic. The DWP serves an average of 30 pounds of nutrient-rich, diabetic-specific staple foods and fresh produce to over 500 individuals every month at 41 distribution sites throughout the community, and 13 home delivery routes.
