Diabetes Wellness

The Diabetes Wellness program (DWP) supports low-income adults living with type 2 diabetes with healthy foods, nutrition education, and connections to community clinics. DWP also provides free diabetes risk screening events for clients attending food distributions in our community.

Program Impact

By increasing access to diabetic-appropriate staple foods and fresh produce, self-management educational tools, and connections to local healthcare providers, Diabetes Wellness has resulted in reduced blood sugar levels, increased vegetable intake, and improved the overall health of participants. The program also reduces the stress associated with food insecurity and managing diabetes, giving our most vulnerable neighbors a chance to thrive.

Community Reach

An alarming 59% of Sonoma County adults are either pre-diabetic or diabetic. Diabetes Wellness serves an average of 30 pounds of nutrient-rich, diabetic-specific staple foods and fresh produce to over 500 individuals every month at 41 distribution sites throughout the community, and eight home delivery routes.
